Sports physicals must be completed every year prior to a student athlete being allowed to practice.

Sports physicals can be scheduled through the DASH office by calling 746-7454 between 8:00am-4:00pm.

Please assist us in scheduling your physical in advance, the office can only do so many in a day and at times will not be able to accommodate last minute calls.
