Health Screening  

New York State Education Department mandates the following health screening on students.

  1. Vision – annually – grades K-3, 5, 7 and 10 and all new entrants  to the district
  2. Hearing – annually – grades K, 1, 3, 5, 7 and 10 and all new entrants to the district
  3. Scoliosis (Curvature of the spine) – annually – grades 5-9   (The above screening will be performed by the school nurse. Parents will be notified of any abnormal findings and appropriate referrals made.)
  4. Physical Examinations of students when they:
    • enter the school district at any level;
    • are in grades K, 2, 4, 7 and 10;
    • participate in interscholastic sports. Results of the examination shall be valid for qualifying a student’s participation for a period of 12 consecutive months;
    • need working papers;
    • are referred by/to the Committee on Special Education;
    • are deemed necessary by school authorities to determine a child’s education program.

    The physical examination may be performed by a student’s primary healthcare provider or the school’s nurse practitioner or physician. If performed at school, parents will be notified in writing of any condition found which may require additional attention. It will be the student’s responsibility to bring in a copy of the physical exam if it was done by their primary healthcare provider.
